We respect your privacy. We do not sell any personal information to any third party such as direct marketing organisations. We only collect and store information which is necessary for the functioning of this website and the medical services provided by Dr. Talia. Your medical information may be emailed electronically, faxed or posted to other practitioners involved in your care, medical or allied health. In most cases correspondence is also sent to the patient.
Security of information MOG has technology and security measures aimed to protect personal and health information, whether in paper-based or electronic form, from misuse, loss, unauthorised access or unauthorised disclosure.
However, you should be aware that there are risks in transmitting information to us online. If you are concerned about conveying sensitive material to us over the Internet, you might prefer to contact us by telephone or mail.
Access to Information
In accordance with the Freedom of Information Act 1982 (Vic) you have the right to request access to your personal information held by MOG. If there is information in the record which is incorrect, you have the right to request that it be corrected. Requests for access to medical records can be made in writing.
Our Terms:
Patients are advised that Dr Talia does bill Medicare direct for Patient Accounts. All patients will receive an Account for professional services provided.
Accounts are typically itemised at the AMA recommended fee level. The AMA recommends a fee schedule based on what is fair and reasonable, and adjusted for inflation, but patients are reminded that these are beyond the standard rebate provided by Medicare and the health insurance companies. Other arrangements can be discussed with your Doctor.
WorkCover and T.A.C patients must provide correct details of the organisation accepting liability for payment of services including their employer, insurance company and claim number before treatment is undertaken.
You will be given your account after your consultation at which time settlement would be appreciated. Mastercard and VISA credit facilities are available.
The terms of the contract are settlement of all consultation accounts on the same day and surgical accounts at least one week prior to surgery.
Please note if your attendance is for a second opinion you should have all correspondence and investigations available from other clinicians. There MAY be an increased charge for second opinions.
Acceptable Use
