Ankle Fractures

As a result of an accident, patients can suffer ankle fractures or dislocations. This can range from a simple twisting injury right through to a car accident. The tibia (shin bone) and fibular (bone on outside of ankle) can be injured, as well as the surrounding ligaments and tendons.

Most ankle fractures are immediately painful and swollen and will require a plaster cast or moonboot with a period of non weight bearing on crutches. Some ankle fractures are deemed stable and are suitable for treatment without surgery.

The ankle joint is made up of three different bones. An ankle fracture can be simple, affecting only one bone, or complex, involving many parts of the ankle joint. X-rays help determine if surgical or nonsurgical treatment is needed.

Proper alignment of the bones is needed for a broken ankle to heal. The type of fracture will help determine which treatment is best. Some ankle fractures can heal with a cast or boot, while others may require surgery to stabilize the bones.

Treatment options:

Non operative treatment

  • Weight bearing in a moonboot – typically for 6 weeks

  • Physiotherapy to strengthen and preserve range of motion

Surgical treatment is recommended for those fractures with are deemed unstable and therefore unsuitable for non-operative management. Each fracture is different and requires an individualised approach.

Metal plates, screws, and rods can be used to hold the broken bones in place to stabilize an ankle fracture. These may be positioned on the back, outer, or inner sides of the ankle joint depending on where the ankle is fractured.

NB: some types of ankle fractures, called pilon or plafond fractures are extremely severe types of ankle fractures, which are more severe injuries than what is discussed here. (see below)
